Maternity Care Management

Our country is experiencing a maternity care crisis with the highest maternal mortality rate among industrialized countries. Since 2000, the maternal mortality rate has risen nearly 60%, making it worse now than it was decades earlier. More than half of these deaths are preventable. This crisis is especially prominent for women of color, who have preterm birth rates 50% greater than average and are three to four times more likely to die during childbirth.

Lucina, a Unified company, has a proven track record in population health, improving patient engagement, and addressing gaps in care to connect families to resources in their communities. Lucina’s support in addressing these patient needs has resulted in a 10% reduction of preterm and low birth weight babies, 2.3 fewer days in the NICU, and a $1,000 savings per birth. Providers supported by Lucina can do this because Lucina’s technology helps identify, profile, assess, connect, and track at-risk mothers.

Lucina’s proprietary database uses an algorithm that identifies 7,000 risk factors while considering prominent social determinants of health. The resulting data helps Lucina find 98% of moms before delivery and 72% in the first trimester, allowing for earlier intervention and access to care that will prevent or limit adverse outcomes, improve patient engagement, and address racial disparity gaps.

Lucina has partnered with innovative Medicaid health plans to advance care. It has grown from supporting 1,000 deliveries in 2020 to more than 100,000 in 2022. In the past year, Lucina’s relationships with health plans have expanded to offer Lucina’s solutions to more than five million health plan members. Lucina currently helps professionals support mothers in 17 states and will add an additional nine markets in the first half of 2023.
